1. MC Authority
2. W 9-Form
3. Certificate of Insurance
4. Notice of Assignment (If you are using factoring)
Qualifications
•
Valid Class A CDL with 1 year of driving experience
•
6 months of verifiable OTR experience in the last 3 years
•
Flatbed Drivers need 6 months of flatbed experience with load securing with chains, straps and tarping
•
Tractor that passes a DOT Inspection
•
Must be 21 years old
