Position Summary: The Social Services Assistant plays a vital role in enhancing the well-being of our residents by assisting with care planning, conducting assessments, supporting discharge processes, and acting as a liaison between families, agencies, and the care team. This position helps ensure that residents receive the emotional, social, and practical support needed for quality care and smooth transitions.
Key Responsibilities: - Attend and actively participate in interdisciplinary care conferences.
- Perform ARD (Assessment Reference Date) assessments in compliance with regulatory standards.
- Conduct routine departmental audits and assist in compliance tracking.
- Provide case management services, ensuring residents’ needs are being met holistically.
- Work closely with families, POAs (Powers of Attorney), and guardians to address concerns and communicate care plans.
- Assist with discharge planning, coordination of services, and communication with external providers or agencies.
- Set and coordinate appointments for care conferences and family meetings.
- Support residents in submitting grievances and help mediate/resolve concerns respectfully and efficiently.
- Advocate for residents' rights and needs within the social service system
- Help residents connect with community resources and services, including food assistance, transportation, and mental health support
- Maintain detailed and confidential documentation per state and federal regulations. Qualifications:
- High school diploma or equivalent required; associate or bachelor’s degree in social work, human services, or a related field preferred.
- Experience in a skilled nursing or long-term care setting preferred.
- Strong interpersonal and communication skills, with the ability to work with a variety of people under sometimes sensitive circumstances.
- Familiarity with MDS/ARD assessments and care conference protocols is a plus.
